1. Download and complete the supportive services intake form.

2. Scan and email the intake form, along with all required supportive documents to epa@cincy-caa.org.

WHAT WE OFFER

EVICTION PREVENTION ASSISTANCE (Policy revision date: April 28, 2020)

Eligibility Requirements: Eligibility is based on 200% of the Federal Poverty Guidelines.  Must have one of the following requirements: 

  • Children ages 17 and under
  • Senior citizens 60 years or older
  • Veteran or a Disabled person.  
     

What you need:

  • Valid Driver’s License or State Identification (must be a resident of Hamilton County, Ohio)
  • Social Security Card for all members in the household
  • Notice to Leave the Premises or Court Order Eviction with your name and address
  • Proof of Income for the last 30 days for all household members
    • (If employed, all paycheck stubs for the last 90 days are required)
  • Statement of hardship (written reason for the requested assistance)
  • Sufficient income to pay future rent
  • Housing HAP Contract if applicable
  • Benefit amount per year may vary case by case

What you need to know:

  • New client appointments (referral, same-day and standby) will only be held on Mondays, Wednesdays and Thursdays of each week.
  • All same-day, standby and referral appointments are limited to households with children (17 and under), seniors (60+), veterans, or disabled persons.
  • Limited slots (6 per day) available for same-day call in appointments (both rent & water). This will be scheduled on a first come, first served basis.
  • Program benefit is limited to one (1) month's rent and applicable late fees.
  • Standby or waitlisted does not guarantee your appointment. Individuals may come and wait to see if an appointment is a no show (15 minutes past appointment time). Standby customer is then eligible to take the appointment slot.

WATER BILL ASSISTANCE (Policy revision date: April 28, 2020)

Eligibility Requirements: Eligibility is based on 200% of the Federal Poverty Guidelines.  Must have one of the following requirements:

  • Children ages 17 and under
  • Senior citizens 60 years or older
  • Veteran or a Disabled person). 

What you need:

  • Must meet income requirements of 200% of Federal Poverty Guidelines (FPG)
  • Must be at least 18 years old
  • Valid Driver’s License or State Identification (must be a resident of Hamilton County, Ohio)
  • Social Security Card for all members in the household
  • Proof of Income for the last 30 days for all household members
    • (If employed, all paycheck stubs for the last 90 days are required)
  • Current Water Bill showing primary applicant's name and address on the bill
  • Past Due Water Bill Notice or the Disconnection Notice
  • Statement of hardship (written reason for requesting assistance, i.e. receipts of unforeseen occurrences)
  • Sufficient income to pay future water bill
  • Benefit amount per year may vary case by case
