Supportive Services


1. Download and complete the supportive services intake form  & Release of Information Authorization form.

2. Scan and email the forms, along with all required supportive documents to epa@cincy-caa.org.

Here's what you'll need to submit with your application:

  • Eviction notice or past due renal statement.
  • Utility shut-off notice or past due utility statement.

 

WHAT WE OFFER

EVICTION PREVENTION ASSISTANCE (Policy revision date: April 28, 2020)

Eligibility is based on 200% of the Federal Poverty Guidelines. You must meet one of the following requirements: 

  • Children ages 17 and under
  • Senior citizens 60 years or older
  • Veteran or a Disabled person.  

WHAT YOU NEED:

  • Valid Driver’s License or State Identification (must be a resident of Hamilton County, Ohio)
  • Social Security Card for all members in the household
  • Notice to Leave the Premises or Court Order Eviction with your name and address
  • Proof of Income for the last 30 days for all household members
    • (If employed, all paycheck stubs for the last 90 days are required)
  • Statement of hardship (written reason for the requested assistance)
  • Sufficient income to pay future rent
  • Housing HAP Contract if applicable
  • Benefit amount per year may vary case by case

WHAT YOU NEED TO KNOW:

  • New client appointments (referral, same-day and standby) will only be held on Mondays, Wednesdays and Thursdays of each week.
  • All same-day, standby and referral appointments are limited to households with children (17 and under), seniors (60+), veterans, or disabled persons.
  • Limited slots (6 per day) available for same-day call in appointments (both rent & water). This will be scheduled on a first come, first served basis.
  • Program benefit is limited to one (1) month's rent and applicable late fees.
  • Standby or waitlisted does not guarantee your appointment. Individuals may come and wait to see if an appointment is a no show (15 minutes past appointment time). Standby customer is then eligible to take the appointment slot.

WATER BILL ASSISTANCE (Policy revision date: April 28, 2020)

Eligibility is based on 200% of the Federal Poverty Guidelines.  You must meet one of the following requirements:

  • Children ages 17 and under
  • Senior citizens 60 years or older
  • Veteran or a Disabled person). 

WHAT YOU NEED:

  • Must meet income requirements of 200% of Federal Poverty Guidelines (FPG)
  • Must be at least 18 years old
  • Valid Driver’s License or State Identification (must be a resident of Hamilton County, Ohio)
  • Social Security Card for all members in the household
  • Proof of Income for the last 30 days for all household members
    • (If employed, all paycheck stubs for the last 90 days are required)
  • Current Water Bill showing primary applicant's name and address on the bill
  • Past Due Water Bill Notice or the Disconnection Notice
  • Statement of hardship (written reason for requesting assistance, i.e. receipts of unforeseen occurrences)
  • Sufficient income to pay future water bill
  • Benefit amount per year may vary case by case

PROJECT LIFT

The Community Action Agency is proud to have a partnership with Project Lift. Project Lift is a unique public/private partnership designed to improve the economic self-sufficiency for families in Hamilton County, Ohio.

Project Lift works to improve economic self-sufficiency by partnering with families working towards self-sufficiency and sponsor agencies supporting families in their efforts. Project Lift also forms strategic partnerships with others to increase access to needed services and supports. 

Project Lift partners with families who meet the following criteria:

  • Live in Hamilton County, Ohio
  • Be responsible for a child (physical, legal, or financial)
  • Working or on a viable path to employment
  • Agree to work with a sponsor agency case manager/coach
  • Agree to share income, employment, and barrier data for at least one year
  • Have a household earned income below 200% of the Federal Poverty Line or Approximately $51,500 annually for a family size of 4.
